Consent form for images
One of the most interactive sessions of the BSCCP is the MDT or case study sessions. We would like members to collect and submit useful images and case studies for the website so that others can share in the learning. It is good practice that images used are used with the consent of the woman for use on the website
The following file is a suggested generic form for recording consent for imaging. Local trust policies may vary throughout the UK and the BSCCP has no formal position on whether consent is obtained in verbal or written form as long as consent is recorded. You may wish to and use it for your practice.
